Issues with Replying to a topic

 I have been experiencing a few issues attempting to reply to topics. I am getting web server error messages. For example, the Quick Reply caused this message to be displayed;

Not Found

The requested URL /forum/newReply/852 was not found on this server.

Additionally, a 404 Not Found error was encountered while trying to use an ErrorDocument to handle the request.
